Accessibility Needs Questionnaire

Accessibility Needs Questionnaire

The City of Rancho Cucamonga is currently in the process of updating its Americans with Disabilities Act (ADA) Transition Plan. This Plan will be completed in three phases, the first one focusing on City owned and operated buildings. The Plan helps the City assess barriers to accessibility, describes how to remove them and provides a prioritization of barrier removal that can be reasonably implemented within the time frame of the Plan.

While formal assessments of our buildings are being conducted by ADA Site Assessment Professionals, we would also like to hear about your experience with potential barriers and how they affect you and your ability to use our facilities.

We value your opinions and thank you for taking the time to provide us with this very important feedback.
