Neighborhood Matching Grants program guidelines:

Forms you will need to complete an application:
- Budget-Volunteer Hours Worksheet (see sample worksheet)
- Volunteer Pledge Sheet - use this form to document the hours pledged toward your match requirement
- Optional: NMG Contribution Template - use this form to document inkind or cash contributions toward your match requirement
- Optional: Volunteer Hours Tracking Sheet / Volunteer Hours Sign-In Sheet - use to document pre-application activities, up to 20 hours
- Optional: Property Owner Permission Template - use this form to show approval for placing your project on private property your org does not own
- Optional: Neighborhood Board Retreat "Developing Your Ideas" Worksheet - upload your completed worksheet to receive $1500 credit toward volunteer hours
Upon grant approval, use this form to track volunteer hours and submit with your follow-up reports:

- Volunteer Hours Tracking Sheet (electronic version, no signatures - use this for virtual meetings or events)
- Volunteer Sign-In sheet (traditional version, signatures needed - use this for in-person programs or events)
