2021 MAYOR'S MENTORING ALLIANCE (MMA) AWARDS

NOMINATION APPLICATION

If you wish to nominate a deserving individual or organization that has made a difference in the lives of Charlotte children and youth through commitment to mentoring, please complete the following application. Nominations for the 2021 Mayor's Mentoring Alliance Awards will close at 11:59 p.m. on Monday, November 30, 2020.

Nomination Eligibility Criteria

  • Individuals and organizations nominated must live or work in and serve the residents of Charlotte, North Carolina.
  • Nominees must not have received an MMA Award in the same (or similar) category within the past two (2) years.
  • Nominations must be submitted for work done by individuals and organizations within the year 2020.
  • Organization nominees should be registered on The Locator by the time their application is to be considered. There is still time to register! For more details on how to register your organization, please visit www.charlottenc.gov/mma.

Please complete all fields in the award for which you wish to nominate. You may nominate for more than one award. All fields are required unless otherwise noted.
