Fish Carcass Reporting Form

Poor environmental conditions can sometimes lead to events known as fish-die offs. Reporting dead fish helps the Washington Department of Fish and Wildlife to track problems as they arise and take appropriate action to protect imperiled fish stocks.

If you would like to report a suspected oil spill or pollution release, don't use this tool; please call 1-800-OILS-911 or visit the Department of Ecology's reporting page. If you suspect poaching, file a report online or call 877-933-9847.

Where was the carcass located?

Select "add point" to drop a pin at the approximate location where you discovered the fish, and include a comment if desired.

Enter the date (MM/DD/YY) and time (XX:XX AM/PM) you discovered the carcass(es).

What type of fish did you observe?

For example: sturgeon, salmon, shad, etc. If more than one species, separate using commas. If you don't know, enter "unknown."

How many dead fish were observed?

Was it just one carcass, or were there many? If many, use your best judgment to estimate the number.

What was the length or size of the carcass that you observed?

If needed, estimate by putting an object of known size next to the carcass. If there are many dead fish, are they similar in size, or a variety of sizes?

Please provide any additional details and upload any photos you took of the carcass(es).
